There is nothing to opt out of here

Some websites offer a choice to opt out of the sale or sharing of your information, because they sell data to brokers or hand it to advertising networks. This site runs no advertising and no analytics, and does not sell or share your information with anyone. There is no toggle to offer here, because there is no such activity behind one.

What you can ask for

You can still ask us about the information we hold. Specifically, you can ask us to:

• Tell you what information we hold about you.
• Correct information that is wrong.
• Delete information we hold about you.
• Send you a copy of the information we hold.
• Stop sending you marketing email.
• Share a copy of your chat history with you.

Use the email address at the end of this page, or see the contact page for other ways to reach us. Include the email address or phone number you used to contact us, so we can find the right record. Before we release information, we may need to confirm your identity.

State privacy rights

Residents of some states have additional privacy rights beyond what is described above. If that applies to you, the request process is the same one described above.

Care records

Requests about your medical record (history, results, or visit notes) go through the practice rather than through this page. Sign in to your patient portal to reach the practice about a records request.
